If you are seeking admissions into our residential treatment program, then please download and fill out our admittal package. When complete, please email to ccskptadmissions@aol.com, Fax to 423-349-6597, or mail to our address.
To fill out a digital form please right-click on a form below in the "Admittal Package" column, then click "save target as" & save the form to your computer. Once you have completed filling out all of the fields, then save your changes to the document, attach the document to an email & send it to ccskptadmissions@aol.com, or print & fax or mail to our address.
